Scott Crook, PT, graduated in 1987 from the University of Kentucky School of Physical Therapy and has over 27 years of experience as a physical therapist. Scott specializes in sports and orthopedic physical therapy and has worked with all levels of athletes ranging from ballet to rugby however, his expertise is in rehabilitation of baseball injuries. Scott also has extensive experience working with neck and back injuries with people of all ages. He has directed speed training camps and sports specific performance training with emphasis on strengthening areas to prevent injuries as well as improve performance. Scott founded Performance Physical Therapy on January 3, 2011 with support from his wife, Cindy, also a physical therapist.
Cindy Crook, PT
Cindy graduated from the University of Kentucky School of Physical Therapy in 1990. She has worked in various rehab settings including acute rehab, skilled nursing facilities, and home health. Cindy has a passion for working with clients who have had amputations and are ready to start training with their prostheses; training ranges from beginners to those who want to return to running, sport, or advanced activities. She also works with other neurological diagnoses and the geriatric population. Cindy is married to Scott Crook and they have two children, Dylan and Carson.
Erin VanZanten, PT, DPT
Erin is a physical therapist that has over 4 years of experience practicing in Ohio and Kentucky. She attended the University of Kentucky for her undergraduate degree and also received her doctorate in physical therapy from UK. Her background includes outpatient orthopedic rehabilitation, aquatic therapy along with sports and work related injuries at Ohio State University. Erin enjoys yoga and pilates and has an interest in implementing these forms of exercise into rehab for orthopedic injuries. Erin also has training in the practice of Corealign, which is a pilates device that focuses on core stability and strength with all functional activities. She also conducts pitching lessons with young softball players and has a passion for executing proper mechanics for injury prevention. Erin enjoys hiking, golfing and trips to the lake with her husband, Matt, son, Wesley and her two dogs.
Carmen Wurtz, PT, DPT
Carmen received her Bachelor of Science degree in 2008 from Georgia State University in Atlanta, Georgia with a major in Exercise Science. She graduated in May 2014 with a Doctorate of Physical Therapy degree from Emory University in Atlanta, Georgia. Carmen grew up outside Atlanta, GA and lived in Nashville, TN working in outpatient orthopedics before moving to Lexington, KY where she now works at Performance Physical Therapy. She has experience working with the Atlanta Ballet company members and has a special interest in injury prevention for swimmers and the performing arts population. During her free time she enjoys hiking with her husband, daughter and two golden retrievers, playing in her garden and deep sea fishing.
Jordan Barker PT, DPT, CSCS, XPS
Jordan received his Bachelor of Science degree in Pre-Physical Therapy and Exercise science from Asbury University where he played varsity baseball for three years. He graduate in August 2016 with a Doctorate of Physical Therapy from the University of Kentucky. Jordan has been working in outpatient orthopedics since graduating and came to work at Performance for the amazing facility and specific clientele it can host. Jordan has experience working with many different patient populations including athletes of all ages and skill levels, from professional to youth, and the general population. Jordan has specific interests in the integration of strength and conditioning
into the rehabilitation setting to treat all patient populations, more specifically all field athletes, crossfitters, powerlifters, and bodybuilders. Currently, he is seeking finishing his certification in Dry Needling. During his free time he enjoys spending time with his wife Lauren, son Brantley, and goldendoodle, Cali (yes she is named after Coach Calipari). He loves his CATS
Patrick Lloyd, PTA
Patrick graduated from the University of Kentucky in 2009 with a Bachelor of Science in Kinesiology/Exercise Science. He then obtained his license to practice as a Physical Therapist Assistant from Somerset Community College in 2012. Patrick has 9 years of experience in the physical therapy setting at various clinics and rehab centers in the central KY area including outpatient orthopedic, acute care, rehabilitation hospitals, and skilled nursing facilities. He is also currently on the staff at St. Joseph Hospital working PRN providing physical therapy services for post-operative and non-post-op patients, including wound care/dressing changes and cardiac rehab. Patrick enjoys working out, playing basketball, spending time outdoors on the water, golf course, and hiking.
