- What To Expect - Being well-prepared for your appointment will ensure your therapist has all of the appropriate information to best help you. New patients, please print out the below forms, allowing you to be seen quicker versus filling out the paperwork before you are seen. Take some time and visit Our Team to familiarize yourself with your therapist and our staff. Your first visit will consist of a thorough evaluation by one of our Physical Therapists. Your therapist will take measurements and observations of your function and strength. This information, along with your doctor's input, will be used to begin a comprehensive rehabilitative program specifically designed to you. Due to the evaluation process, the first visit may be a bit longer than your subsequent visits.
- Be sure you bring your doctor's script, insurance card and photo identification, and any other information you think may be necessary, i.e. imaging and other test results.
- Patient Forms - In order to help you save time during your first appointment, we encourage you to download and complete the new patient paperwork in advance. If you are unable to complete your paperwork, please arrive 30 minutes before your appointment time to allow sufficient time to complete the necessary paperwork and allow us to review it.
